What should I bring to my office appointment?
Please arrive 20-30 minutes prior to your appointment if you are filling out new patient information in office. You are able to fill it out online on our Patient Portal. You should have received an invite when you made your appointment.
If you have not recieved and invite, please call the office. You can also text the office through our Klara app. The app is found on any app store. This will be the app that will be used for appointment reminders.

Depending on the nature of your appointment, you may want to bring:
- Medication list
- A list of your past medical and surgical history
- Digital copies (not just reports) of previous radiographic imaging (CT, MRI, Ultrasound or X-Rays)
- Drivers license or state issued identification card
- Insurance cards
- A list of questions you may have
- A friend or family member
